To send a case:

1. Fill out and submit our online Rx form or print out Rx form and mailing label.
2. Fill out Rx form.
3. Wrap models, impressions, bites, etc. carefully with tissue or bubble wrap.
4. Securely tape box and affix mailing label.
5. Mail via US Postal service.

We will return your cases using UPS or Airborne service at no cost to you.

You may also call us toll-free and request a Doctor start-up kit which will include boxes, packing materials, pre-paid labels, and Rx forms.
